In this instance, from the book and subsequent TV show Handmaid's Tale, Gilead is the dystopian nation that has overtaken the U.S., in which women have been stripped of reproductive autonomy. Women basically have no rights, no name, no property. They are nothing other than sex slaves owned and coerced by men. Abortion activists have been comparing Gilead to the Trump administration.
