The 12 men breaking the huddle only applies to offense. Most defenses don't huddle anyway. The purpose of the rule is so that the offense cannot deceive the defense.

Very close to not being set, could have been called. Looks like enough on the line, I only count 4 in the backfield. My question is was the whistle blown and the play dead before the helmet was removed? That could have been big trouble.
The thrown helmet should have been called, but I don't know how they would have payed attention to that given what was happening on the field at the time. Besides, can a game end on offsetting penalties? It would probably just be a replay of one more down on the same yard marker.
watch the replay again. you can see the back judge counting UT guys and reaching for flag. ball is snapped before he gets it out, but he is definitely reaching for it, so i think it gets called either way.
In the chaos, Tennessee could've also been called for storming the field with THEIR helmets off. But I do think Hebert's helmet went flying before JJ even got to the ball. Once JJ landed on the ball, the game was "over", so it wouldn't matter.
Both penalties would have been enforced in order of occurrence....half the distance against Tennessee, then 15 yards back against LSU...one last play from the 15 yard, 1 ft. line or so. the NFL don't end on offsetting penalties (or at least didn't). remember 6-7 years ago Giants and 49ers in playoffs? SF botched a snap on last second FG, scrambled and attempted a pass. penalty called for ineligible man downfield, game over. that week, NFL announced the proper call should have been. IR downfield offense, PI defense. penalties offset, replay the down with no time on clock. I don't know if that is the college rule or not though.
